I acknowledge that myself or my child's presence at, and/or my or my child’s decision to attend, prepare-for or participate in any event, show, rehearsal, audition, class, and/or workshop (hereinafter the “Event(s)”) at any Ministry in Motion Inc (hereinafter “Ministry in Motion”) event location are strictly voluntary choices, in spite of the unavoidable risks, hazards and dangers of such activity, including risks to myself or my child’s health and well-being. Myself and/or my child hereby accept any and all risks associated with the Event(s), including the risk of injury or death, and will hold harmless Ministry in Motion Inc, its assigns, officials, representatives, employees, contractors or owners for any injury or damage to myself or my child, or my or my child’s property sustained while participating in, preparing for, or attending the Event(s). At all times when observing, participating in, preparing for or while attending the Event(s), myself and my child (if applicable) agree to appropriately limit activities for my or my child’s level of conditioning, skill, and experience. I and my child agree not to take any unreasonable risks (including the consumption of alcohol or drugs) before or during my or my child’s participation at the Event(s), or to cause any other persons present to take or be exposed to any unreasonable risk or harm. Myself and my child (if applicable) agree to comply with all of the written, stated or customary terms, laws, rules, and conditions for participating in such event(s).
In exchange for Ministry in Motion Inc allowing myself or my child to be present at and/or participate in Event(s), myself and/or my child hereby irrevocably grant to Ministry in Motion Inc and their representatives, licensees, designees, permittees and assigns, the right and authority to film, videotape, photograph and/or record, in any manner or medium, myself or my child in connection with this Event and any other Event(s), and to use such film, videotape, photography and/or recordings (“Products”) in perpetuity, any number of times, for any purpose and in any manner or medium now or hereafter known in connection with the display, distribution, promotion, marketing or sale of Products and/or services by any means and through any channels, including, without limitation, in or on any website, marketing, advertising, promotions and packaging relating to Ministry in Motion Inc.
I and my child hereby authorize Ministry in Motion Inc to enroll myself and my child into their text or email database and send us correspondence as they see fit. I understand that I and/or my child have the ability to opt-out and remove information from the Ministry in Motion Inc databases at our discretion.
I and my child understand that Ministry in Motion Inc reserves the right to substitute speakers, instructors or choreographers if they are unable to be at Event(s) due to scheduling conflicts or any other issues that may arise.
Myself and/or my child understand that acceptance below is equivalent to an electronic signature. Furthermore, I and/or my child understand that an electronic signature or facsimile signature shall be deemed the same as an original signature. Myself and/or my child agree to promptly re-execute a copy of this agreement with an original signature if requested to do so by Ministry in Motion Inc. Per my acceptance below, I have read and understood this agreement and how it applies to myself and/or my child, and have been given ample time to ask any questions. Both myself and my child (if applicable) understand and recognize that there are no cancellations or refunds once a purchase or reservation is made.
